EARtrak Privacy Policy

Scope

This Privacy policy applies to information collected by EARtrak from practices and their clients, to the use of that information for reporting to clinics and for the use of the database for research purposes.

Information Handling Practices

Collection

Information (Client Data) forwarded to EARtrak from clinics is de-identified by the clinic to protect the identity of their patients.

Information (survey responses) forwarded to EARtrak from patients is de-identified with the use of a unique code (assigned by the clinic, and unknown to EARtrak).

The forwarding of the Client data and completed surveys to EARtrak constitutes agreement for inclusion of the data in the EARtrak database.

No personal information is collected that would enable identification by EARtrak of individual patients (compliant with the Privacy Act 1988).

Use and Disclosure

EARtrak only uses the information collected for the purposes for which it was collected – purposes which are related to EARtrak’s functions and activities.

EARtrak does not give identifiable information about individuals or clinics to Government agencies, private sector organisations or research bodies.

EARtrak may, from time to time, participate in research projects with third parties or within the EARtrak organization. The de-identified database secures privacy of patient and clinic information.

Data Security

All steps are taken to protect the information from patients and clinics against loss, unauthorized access, use, modification or disclosure, and against other misuse.

All data is securely backed up off-site.
